I, the undersigned parent/guardian, hereby give permission for my child(ren) listed above to participate in The Almost Overnight Party hosted by the Andrew Jackson School Parent Teacher Organization (“AJS PTO”).
1. Release of Liability
I understand that AJS PTO volunteers will supervise children to the best of their ability. However, I acknowledge that participation in group activities involves inherent risks. I agree not to hold the AJS PTO, its officers, volunteers, employees, Andrew Jackson Elementary School, or the school district liable for any injury, illness, or accident that may occur during this event, except in cases of gross negligence or willful misconduct.
2. Medical Authorization
In the event of an emergency and/or if I cannot be reached, I authorize AJS PTO volunteers to:
Seek emergency medical treatment for my child(ren),
Contact emergency services, and
Provide information to medical personnel on my behalf.
I agree to be responsible for any medical expenses incurred.
3. Behavioral Expectations
I understand that children must follow directions and behave appropriately. If a behavior issue arises, I agree that:
I may be contacted for early pickup, and
My child may not be permitted to attend future events of this nature.
